Abstract     Screening programmes are important medical tools for an early detection of hidden diseases and defects, but only if they are carefully weighed up against possible advantages or disadvantages. The implementation of screening programmes in national health preventive programmes requires a thorough evaluation of their benefits. The validation of each screening programme has to contain an estimation of their conformity to the principles of WHO about screening programmes as well as an evaluation regarding their health efficiency and economic efficacy. Health (selection of quality parameters concerning reproductivity, sensibility, specificity, the predictive value) and economic (CBA, CEA, CUA) analyses have to be accompanied also by considerations about possible ethic and psychological consequences and by a plan of an educational, informational, organisational and financial support for the proposed screening programme. As a rule the implementation of a new screening programme has to be approved by a competent national body (an unbiased multidisciplinary board), which also supervises the execution of accepted and standardised programmes.
