Abstract     Atherosclerosis and hypertension are major risk factors for cardiovascular disease and are closely related to each other. Atherosclerotic plaques have been found even in infancy and advanced sclerosis with reduced arterial diameters is sometimes observed already before puberty. The authors are concentrating on the prevention of atherosclerosis by reduction of serum cholesterol levels and on possibilities for preventing primary hypertension by a wide range of prophylactic measures. A general screening for hereditary hypercholesterolemia has been advocated in different countries. The German working group for pediatric metabolic diseases has recently proposed a scheme for screening and dietary and drug treatment of hereditary hypercholesterolemias. Two forms of strategies have been proposed for preventing hypertension in childhood: the population and the high risk approach. At present, an educational population approach seems to be most promising. The use of new diagnostic techniques (24-hour ambulatory blood pressure monitoring, reliable genetic markers of primary hypertension) might improve identification of children destined for hypertension in adult life.
