Abstract     Acute diarrhoea in infants and small children is treated by replacing fluid and electrolyte losses, giving antimicrobial drugs when necessary, and by providing enough food to meet patient's usual requirements and increased needs due to infection and diarrhoea. Until recently the main emphasis was placed on oral rehydration solution, but in the past few years the interest has been focused on the role of feeding. However, the optimal timing, the choice and the amount of food for providing nutritional treatment are still a matter of strong debate and disputes worldwide. This review is summarizing present approaches to dietetic treatment of acute diarrhoea in infants and small children. The optimal timing of feeding and the role of human milk and solids in acute diarrhoea are discussed. However, special emphasis is placed on the use of lactose-containing versus lactose-free diets, and undiluted non-human milk formulas versus diluted milk.
