Abstract		All emergency conditions request efficient and fast informations about patient's condition of vital functions and circumstances, where the emergency condition occur. We admiss and deliver the patient either in the hospital, when the transport between two hospital units is needed and in terrain, where the emergency is present. Emergency medical technician or emergency nurse has to take all important informations from the patient or his relatives. Information can also be provided from the people on the scene of incident. Important data we collect on the scene and during the transport are: - Location of incident - Approximate estimate of age and sex and number of emergency patients - Present status of the patient, with condition of vital functions and our ascertains we found by examination - Therapy and other precautions we gave to the patient.
