Abstract     The article is a short review of portal hypertension. It discusses the accepted as well as new knowledge on the pathogenesis of the disease syndrome and gives a survey of the etiology of portal hypertension. We are well acquainted with the clinical manifestations of the syndrome. Among them, due above all to their dramatic character and high mortality rate, hemorrhages from esophagogastroduodenal varices are still prominent. The diagnosis of portal hypertension is multilayered. Simple and uninvasive methods are used above all to detect the indirect signs of portal hypertension while the methods of direct pressure measurement are invasive and used less frequently in our everyday practice. Therary is mainly directed towards decreasing portal hypertension and overcoming the ensuing complications. Several approaches are possible, from conservative to semisurgical and surgical ones. None of the methods of treatment can prolong life essentially as compared to the other methods, but some can significantly decrease the number of the most serious complications - hemorrhage, although at the price of another undesired complication, encephalopathy. Transjugular intrahepatic portosystemic shunt (TIPS), a relatively new method, may contribute more as it causes fewer complications and decreases the number of recurrent hemorrhages considerably. But at present there is still no extensive study on the prolongation of life with this method.
