Abstract     Background. Coronary artery disease represents a risk for patients, undergoing elective major surgical procedures. With combination of good clinical judgement and several diagnostic procedures we could individually assess the cardiac risk. The history alone could not detect the underlying disease, responsible for cardiac risk associated wirh major surgery. With nonivasive procedures according to the proposed algorithm (Physical stress test, Stress-ECHO, TI-Dipyridamole, MUGA) we could define the extent of coronary artery disease and detect patients with high risk at major surgery. Conclusions. Good clinical judgement is essential. Objective noninvaive procedures for assessment of cardiac status should be employed in the final medical analysis and recommendations at clinically suspected patients. The goal of cardiac risk assessment should be a consideration of the impending surgery as well as the long term cardiac risk factors responsible for the patients survival.
