Abstract     There is still debate as to the treatment of a neck with no clinical evidence of nodal metastases in patients suffering from carcinoma of the oral cavity and oropharynx. There is a high incidence of occult metastases and the appearance of local metastases following treatment limitad to the tumor leads to sharp drop in survival rates. Imaging diagnostic examinations such as echography, computerised tomography and magnetic resonance imaging have high diagnostic accuracy, but by that mean it is not possible to evaluate whether occult metastases and neoplastic cell in transit are present or not. In an attempt to rationalise treatment of the neck with no clinical evidence of nodal metastases we have evaluated the efficiency of invasive cell grading as a prognostic factor of occult metastases. The invasive cell grading score was evaluated in a pre-surgery biopsy of the invasive border of the carcinoma and postoperatively in the surgical specimen of the neck dissection. A comparison between the invasive cell grading and the condition of the lymph nodes showed that there was a statistically significant correlation between the invasive cell grading category and the presence of local occult metasteses.
Summary     Vprašanje, kako zdraviti bolnika s karcinomom ustne votline in usnega žrela brez kliničnih znakov prisotnosti zasevkov v vratnih bezgavkah, še vedno ni razrešeno. Pojavnost okultnih zasevkov je visoka in če se zdravi samo tumor, pojav področnih zasevkov zelo zvišuje umrljivost teh bolnikov. Slikovne diagnostične preiskave, kot so ultrazvok, računalniška tomografija in magnetna resonanca so zelo natančne, toda z njimi in mogoče oceniti morebitne prisotnosti okultnih zasevkov in manjšega števila tumorskih celic. Z namenom racionalizacije zdravljenja vratu bolnikov brez kroničnih znakov področnih zasevkov smo ocenili učinkovitost ocenjevanja stopnje celične invazivnosti (gradusa) kot napovednega dejavnika za okultne metastaze. Stopnjo celične invazivnosti smo ocenili v bioptičnem vzorcu invazivnega roba karcinoma pred operacijo in v vzorcu med operacijo odvzetega tkiva. Primerjava stopnje celične invazivnosti in dejanskega stanja bezgavk je pokazala statistično značilno povezavo med stopnjo celične invazivnosti in prisotnostjo področnih okultnih zasevkov.
