Abstract     Backgraung. The aim of the study was to assess the diagnostic value of changes in IgG anti Helicobacter pylori (HP) antibody levels during the two year period in dependency of HP eradication. Methods. Between November 1992 and November 1993 we included 33 patients in our prospective study. In the study group we included 16 HP positive duodenal ulcer (DU) patients after the eradication of bacteria. In the control group we included 17 HP positive DU patients which had only H2 blocker therapy. All patients included had at the beginning of the study, 2 month and one year after upper GI endoscopy with four biopsies for histology (Hematoksilin eoyin, Warthin Starry): 2 biopsies from antrum and two from corpus. At the same time and also at 6, 18 and 24 months we took 2 ml of serum which was froyen at - 20 degree C. At the end of the study we analised IgG levels with ELISA test Amrad Hel-p TEST/TM II. which have specificity of 93% and sensitivity of 96%. Positive values are between 50 U/ml and >800 U/ml. Results between 30 U/ml and 50 U/ml are in the grey zone of the test. For the statistical analyse we used hi squer test, Mann Whitney test and Wilcoxon test. A p< 0.05 was considered significant. Results. IgG anti HP antibody were at the beginning of the study very dispersed between < 50 U/ml (2 patients with false negative results) and > 800 U/ml in both groups. Mean values of all IgG anti HP antibody levels in the control group were stable during the two year observation period: 427 U/ml (0 months), 489 U/ml (2 months), 436 U/ml (6 months), 306 U/ml (12 months), 437 U/ml (18 months) and 474 U/ml (24 months). The results were spread between < 30 U/ml and > 800 U/ml during the whole observation period. In the stady group there was a gradual reduction of mean values during the observation period of two years: 492 U/ml (0 months), 293 U/ml (2 months), 167 U/ml (6 months), 92 U/ml (12 months), 93 U/ml (18 months) and 83 U/ml (24 months).(abstract truncated at 2000 characters)
