Abstract     Adjuvant wound treatment is critically discussed. The author believes that a wound usually hea/s by itself and that this should not be disturbed by unproper medical treatment. Ideal micro climatic environment - humidity and temperature is maintained by rational wound dressing. Sterile conditions hinder contamination, while gentle and proper handling prevents mechanical in chemical damage of young connective tissue. During the dressing procedure, clean wounds are irrigated with Ringer solution - disinfectants are generally not used. Purulent wounds should be dressed a few times daily (every 8 hours) with compresses of Ringer solution. The layer of a compress nearest to the wound should never completely dry out. When however we do use antiseptics with heavily contaminated wounds, they are not to interfere with fibroblast growth (we use silver nitrate in 0,2 % solution, Lavasept(C)in 0,1-0,2% solution of20% concentrate, metronidazol gel in bad smelling wounds infected with anaerobes and silver sulfadiazin with burns). Necrotic tissue has to be removed surgically and only exceptionally by the method "wet to dry dressing" for a few days or with enzyme preparations (with necrotic remnants in highly debilitated patients).
