Abstract     L-asparaginase therapy for childhood acute lymphoblastic leukemia and non-Hoodgkin's lymphoma may be associated with toxic reactions of varying frequency and severita. The frequency of hyperglicemia in 169 children who received L-asparaginase as part of their induction therapy was analysed retrospectively. Two patients (1,2%) developed diabetes mellitus within one week following the first application of L-asparaginase. Hyperglicemia was associated with decreased plasma insulin level in one case, while in the other, diabetes can be associated with a partial insulin resistance and diminished beta cells secretory capacity. Diabetes resolved in both patients after 17 days of insulin therapy. OGTT performed after their transient diabetes revealed no impaired carbohydrate metabolisem. Although transient diabetes mellitus in patients treated by L-asparaginase is supposed to be due to impared byosynthesis or release of insulin, other mechanisms could be involved in the pathogenesis of this rare disorder of carbohydrate metabolism.
