Abstract     Blood diseases with neutropenia are prone to infections with fever. One must recognise diseases with fever and neutropenia at the same time and the combination of two diseases, one causing neutropenia, the other fever. Neutropenia is defined as neutrophil blood count less than 1.8 X 109/I. Severe neutropenia causes susceptibility to infections. The risk of infection is related to neutrophil blood count, duration of neutropenia and other factors. Several pathogenetic mechanisms are operating in neutropenia. In a patient with severe neutropenia and fever there is usually blood disease with an infection. Other rare possibilities are discussed. Fever in severe neutropenia is usually a sign of infection in patients with blood disease. There are other rare possibilities for the combination of the two signs, fever and neutropenia.
