Abstract     Infections are the leading cause of death in neutropenic and immunocompromised patients. In addition to routine prophylaxis, treatment with antibiotic, antifungal and antiviral drugs, and with granulocyte-macrophage and granulocyte colony-stimulating factors should be instituted in these patients. As shown by a recent study, quinolone prophylaxis most effectively decreased the incidence of gram-negative infections. In patients receiving quinolone prophylaxis, however, overgrowth of resistant gram-positive bacteria was documented. Fluconazole is the most effective antifungal agent for preventing superticial and disseminated candidiasis. Patients receiving this prophylaxis, however, are at increased risk of developing infections caused by fluconazole-resistant fungi. A judicious use of anti-infective therapy in patients with impaired neutrophil and macrophage production improves the quality of life and lowers the cost of hospital treatment.
