Abstract     Immediate reconstruction following segmental mandibulectomy is an accepted surgical technique. In the past two years, 14 patients at our department underwet reconstruction of the mandible with a free fibular flap directly following segmental surgical excicion. Ten patients had squamous cell carcinoma of the oral cavity, two had ameloblastoma, one had adenoid cystic carcinoma and one had osteoradionecrosis with pathologic fracture of the mandible. In eight patients, we used a free osseous fibylar flap in combination with a free radial forearm flap. in six patients, a free osseocutaneous fibular flap was employed. One free fibular flap failed in the immediate postoperative period and could not be revascularised. It was immediately replaced with a reconstruction plate and a free radial forearm flap. Two free fibular flaps failed after radiotherapy. In both cases, the exposed bone was removed without secondary reconstruction. All the complications occurred in mandibular defects involving the anterior mandibular arch. Eleven patients in the series had malignant disease. Eight of them are still alive and free of disease. Four patients are able to eat normal food, the others take a semisolid diet. All the patients have articulate speech. With this method a good functional and aesthetic state can be restored in most patients.
