Abstract     A new transclinoid-transsellar-transcavernous (3-T) approach to basilar tip aneurysms in presented. In its initial version this approach was already presented in 1987 by the senior author. In the next few yers the 3-T approach was further improved, so that in its present version it offers a much safer and easier method of treating basilar tip aneurysms than the original 3-T approach. The individual steps of the surgical procedure are described in detail. The 3-T approach to basilar tip aneurysms provides the required space around the aneurysm, which is mandatory for safe proximal control as well as for good visualization of the whole aneurysm, and allows for dissection and clipping of the anurysm. This approach does not exclude the use of any other approaches, on the contrary, it allows for combination of various approaches to different degrees, depending upon the size and position of the basilar tip aneurysm. Final important point to be mentioned is that the 3-T approach no longer requires so extensively practised retraciton of the brain during the operation. A series of 47 patients with a basilar tip aneurysm were treated surgically using this approach. The final outcome is as follows: out of 47 patients treated, 33 (70.2%) are sumptom-free, 10 (21.3%) have ccertain symptoms and/or neurological deficits, and 4 (8.5%) died after surgery.
