Abstract     The extended excision of the ampulla of Vater was first described by the author in 1992. In this article the principles of operative technique of ampullary excision are presented and two kinds of reconstruction are described. In a six year period eight patients were treated using this procedure. The first patient had ampullary carcinoma, three patients had ampullary adenomas with displasia (II, III), two patients had ampullary adenomas with carcinomatous changes, one patient had carcinoma of the duodenum and one patient had adenocarcinoid of the ampulla. Effects of the treatment are critically discussed assuming that the operative procedure has proved to be simple and postoperative course uneventful. Late results of extended excision of the ampulla of Vater were good in patients with adenomas with dysplasia, adenomas with carcinomatous changes and with adenocarcinoid. Recurrent disease had been found in a patient with ampullary carcinoma, who had died three years after the procedure and in a patient with duodenal carcinoma, who had been reoperated after three years and who is still alive after Whipple resection. We conclude that the method is suitable for adenomas and low grade malignant tumors, but it can be used also in high operative risk patients having ampullary carcinoma instead of local transduodenal excision of the ampulla.
