Abstract		Pattern of the onset, temporal progression and extent of autonomic impairment is different in idiopathic Parkinson's disease (PD) and in multiple system atrophy (MSA). Constipation may be an initial problem in both conditions. The other autonomic dysfunctions are usually mild in PD. They appear mostly in the advanced stages of the disease. Urinary dysfunction and orthostatic hypotension may predominate in the clinical picture from the beginning in MSA. Early impotence, anhidrosis, respiratory and sleep abnormalities strongly support this diagnosis. Further laboratory testing will contribute to a better distinction of different autonomic syndromes. Currently, they are only of supportive diagnostic value in individual patients. Systemic diseases and several drugs can play an important role in autonomic symptoms and signs. Many nonpharmacological measures are beneficial in alleviating symptoms and some effective drugs for symptomatic treatment are available.
Summary		Parkinsonova bolezen (PD) in multipla sistemska atrofija (MSA) se razlikujeta po začetih simptomih, po njihovem časovnem razvoju in po razširjenosti motenj avtonomnega živčevja. Zaprtje je možno v začetnih obdobjih obeh bolezni. Ostale avtonomne motnje so pri Parkinsonovi bolezni navadno blage in se javljajo v poznih fazah. Mikcijske motnje in ortostatska hipotenzija so lahko pri MSA prevladujoče težave od samega začetka. Zgodnji podatki o impotenci, anhidrozi, respiratornih motnjah in motnjah spanja močno podpirajo diagnozo MSA. Natančnejše razločevanje avtonomnih sindromov omogoča laboratorijsko testiranje, ki pa pri individualnem bolniku zgolj podpira klinični vtis. Pri obravnavi avtonomnih motenj se zavedamo pomembne vloge sistemskih bolezni in zdravil. V terapiji poleg mnogih nefarmakoloških ukrepov uporabljamo tudi nekatera učinkovita simptomatska zdravila.
