Abstract     The aims of the present synthesis report were as follows: examination of the quality and comparability of data; actual comparison of data on treatment demands in the various cities. With the synthesis the following fundamental problems must always be taken into account: the (lack of) uniformity of terms, instruments used and methods and evaluation of data collection; the existence of cultural and political differences between individual cities and countries; the existence of different structures with regard to treatment systems. In summary, the following principal results emerged: Data are, in some cases, of very good quality and can easily be compared, although improvements in quality and comparability are both necessary and possible. Various efforts must be made towards further standardising survey and evaluation methods. The data presented provide a very good basis for interpretations and in-depth analyses. The most important differences between the various cities are: large fluctuations in the proportion of first treatment demands; very big differences with regard to current living status (living with parents or extended family); significant differences in the proportion of persons in regular employment; strong differences in age distribution; heroin and other opiates are the predominant primary drugs in all cities; wide variations in the routes of administration, with the trend away from the injection of drugs towards smoking through aluminium foil, "chasing the dragon" (heroin); virtually no differences in the age of first time use of primary drug (around twenty).
