Abstract		Background. The article gives a clear presentation of chronic critical limb ischemia (CLI). There is a full definition as well as a detailed description of diagnostic criteria follwed by a review of therapeutic procedures and different possibilities for treatment. Conclusions. CLI has proven to be a very serious condition, not only a threat to the patient's limbs, but to their lives as well. Dealing with the problem involves an interdisciplinary approach. First, it is imperative that all possibilities of revascularisation are considered, since medications have had only limited successs. Although some new revascularisation procedure are available, it is necessary, in more than one fourth of the cases, that the affected limb is amputated, which has grave consequences, both medical and psychosocial.
Summary		Izhodišča. V prispevku je pregledno predstavljena kritična kronična ishemija udov (KIU). Nadrobno so podani diagnostični kriteriji in definicija KIU. Patofiziološkemu opisu sledi opis terapevtskih posegov in različnih možnosti zdravljenja. Zaključki. Avtor ugotavlja, da je kritična kronična ishemija uda zelo resno bolezensko stanje, ki ne ogroža zgolj prizadetega uda, ampak pogosto tudi bolnikovo življenje. Obravnava bolnika s KIU zahteva interdisciplinarni pristop. Pri zdravljenju KIU je potrebno najprej proučiti vse možnosti za revaskularizacijo, kajti zdravilsko zdravljenje je le malo učinkovito. Kljub novejšim možnostim revaskularizacijskih posegov je pri več kot eni četrtini bolnikov s KIU potrebna amputacija uda, ki ima hude zdravstvene in tudi psihosocialne posledice.
