Abstract     The role of IGF-I in low ovarian response to ovarian stimulation by exogeneous gonadotrophin has not been clarified yet. Our study was designed as to evaluate the role of IGF-I in low ovarian response. Forty randomly selected "low-responders" (group "LOW") were compared with follicular fluid and serum IGF-I concentrations to 120 normal responding patients (group "NORMAL") in IVF treatment. Follicular fluid IGF-I concentration did not differ significantly between the two groups (235.27 micro g/l in group "LOW" and 233.48 micro g/l in group "NORMAL"; p < 0.05). The serum IGF-I concentration was significantly higher (343.74 micro g/l) in group "LOW" regarding the control group "NORMAL" (269.13 micro g/l; p < 0.05). Thus, we could propose that an elevated IGF-I level could represent an indicator of occult ovarian failure as well as an indicator of ovarian resistance for gonadotrophin stimulation in patients with normal basal FSH levels.
