Abstract | The subjects of this prospective one-year study were 93 patients of both sexes, various ages (-x=49 years) and various dialysis duration (-x=52 months). Thirty-one of these patients with serum Ca concentration 2.7 mmol/L, P concentration under 1.8 mmol/L and PTHi concentration over 65 pg/ml (Group 1) were receiving calcitriol in a dose of 0.25 micro g/day. The control group consisted of patients without calcitriol administration. Group 2 had normal serum Ca (-x=2.47 mmol/L) and P (-x=1.69 mmol/L) concentrations, and group 3 had a high CaXP product - >5.6 (Group 3). The dynamics of average plasma P, Ca, AP, PTHi concentrations and X-ray changes in the various groups of patients at the beginning of the investigation and one year later were compared. At the end of the one-year treatment period there was a statistically significant increase in Ca concentration (p<0.005) and in P concentration (p<0.005) for group 1. There was also a statisticaly significant decrease in average PTHi concentrations in group 1, as compared to group 3 (p<0.005). The X-ray changes characteristic of secondary hyperparathyroidism progressed more slowly in group 1. These findings show that peroral calcitriol in small doses is effective in increasing Ca and concentrations and preventing the further secondary hyperparathyroidism. | |
