Abstract     Chronic obstructive pulmonary disease (COPD) is consequence of smoking, environmental and occupational exposure to dust and gases and in minor proportion to hereditary factors. Role of other potential risk factors (viral infection) is under investigation. Bronchial hyperresponsivness is important factor and suggests possible interlinking between asthma and COPD (Dutch theory). No firm evidence opposing British theory of two different diseases with different pathogenesis- asthma allergy and COPD inflammmation due to smoking exists. Better insight into pathogenesis is prerequisite as to more succesful treatment and eventual prevention of this common disease.
