Abstract		A review of theophylline mechanisms of action, its effects on respiration, numerous adverse events and the role of drug in the treatment of COPD are presented. Theophylline may be given in acute exacerbations of COPD in combination with anticholinergics and beta2 agonists. Plasma levels should be measured because of its narrow toxic: therapeutic ratio.
