Abstract     Because airflow obstruction in COPD is partially reversible, aggressive bronchodilator treatment is appropriate. Bronchodilator treatment in COPD is only symptomatic. it diminishes breathlessness, improves exercies tolerance and enhances quality of life but doesn't change the natural course of the disease. Except in mild disease inhaled bronchodilators should be used on regular basis and not "as needed". Ipratropium bromide is the treatment of choice because it has a pronounced bronchodilator effect and apparently no side effects. Treatment should begin with a single inhaled broncodilator. A bronchodilator potency of combination of ipratropium bromide and beta agonist is greater than the potency of a single drug. Bronchodilator treatment of COPD should follow the ERS statements.
