Abstract     A physical injury notification is a form, which is regarded by the state prosecutor as only an orientational basis for submitting a possible request for starting a criminal investigation procedure, while it serves for the police as the basis for submitting a criminal charge. The document includes all elements defining physical injuries according to the criminal legislation and is not regarded by the state prosecutor and the Ministry of Internal Affairs as an expert opinion of the physicians who filled out the form. Superficial completion of the form either due to negligence or even lack of knowledge often leads to erroneous conclusions and judicial decisions if the document is used as the only medical documentation without the appropriate expertise.
