Abstract     Mechanical ventilation is one of the most important interventions in the prehospital care. One of the main reasons for the use of mechanical ventilation is increased work of breathing and increased oxygen consumption. Decrasing work load and oxygen consumption improves tissue oxygenation. Improved tissue oxygenation might decrease the risk of the multiple organ failure. Indications for the mechanical ventilation includes low SaO2, high breathing frequency and other signs of the thoracal injuries should prompt care provider to consider mechanical ventilation. In prehospital care controlled ventilation is used. The care provider should ensure that the patient ventilatory needs are taken care of, by setting adequate tidal volume and frequency avoiding excessive inspiratory pressure and barotrauma. The patient should be sedated and if necessary paralysed. Transport respirators of different designs are used. Most of the respirators are constant flow generators which are time cycled. For the patient monitoring beside clinical findings pulse oxymeter or even better capnograf is used.
