Abstract     Ageing is a universal, progressive physiological phenomenon, accompanied by degenerative changes in structure and function of organs and tissues. The integrated organ system function is diffrent from one elderly patient to the other. By the elderly patient with concomitant diseases the functional organ reserve is decreased. Trauma represents a great stress for the elderly individual. His homeostasis is very susceptible: trauma can bring him to the very edge of his ability for compensating unbalance: hypovolemia, hypoxia. Different concomitant diseases diminish the possibility for survival, which depends mainly on the type of trauma, on patient's response to it, on the adequacy and time of therapy, on the quality to the transport to the hospital. The goal of the therapy is to correct or prevent hypovolemia and hypoxia, which are very dangerous for the elderly patient. BLS and ATLS improve survival. The "Golden hour" is important to the treatment of the trauma of the elderly as well. The age does not determine the decision for resuscitation and does not influence mainly the survival. Out of hospital or field therapy is still of greatest importance.
