Abstract		We describe two critically ill patients who suffered acute respiratory insufficiency associated with development of tension pneumothorax and acute airway obstruction. Basic pathophysiology, clinical approach, differential diagnosis and treatment are discussed. Early clinical diagnosis of tension pneumothorax and airway obstruction with emergency decompression, reintubation and oxygenation are of vital importance.
