Abstract     A 78-year old woman with history of a moderate chronic biventricular heart failure suffered a cardiogenic shock after the first 80 mg oral dose of verapamil. She was resuscitated with artificial ventilation, dobutamine, norepinephrine and intravenous calcium gluconate. After 6 hours of treatment the patient gradually regained consciousness, breathed spontaneously, heart rate and blood pressure increased, and diuresis appeared. Toxicological analysis revealed unexpectedly high plasma verapamil concentration. We conclude that particularly in patients with pre-existent bivevtricular heart failure a single oral therapeutic dose of verapamil can be life threatening.
