Abstract     Hemiplegic muscles often show abnormally fast fatigue when stimulated through their nerves. Single fibre EMG with axonal microstimulation was used to examine the possible contribution of neuromuscular transmission impairment. The study was performed in normal and paralysed limbs of 24 hemiparetic patients with a recent ischemic stroke. The mean jitter values were mildly but definitely abnormal on the affected side in 18 patients, suggesting some degree of transsynaptic lower motor neuron dysfunction during early stages of upper motor neuron paralysis. This degree of impairment, however, does not seem to play any significant role in the abnormal fatigue of muscles affected with upper motor neurone weakness.
