Abstract     Twenty cases of large defects in long hollow bones were treated at the Department of Traumatology, Ljubljana, from 1983 to 1995. External fixation of bone fragments obtained by corticotomy was used, after which segments of live bone were gradually transferred into the region of the bone defect. Ilizarov-type external fixators were used in 14 cases, while in 6 cases we employed the tubular external fixator onto which a special home- constructed Ljubljana-type traction device was attached. Simultaneously, the defect was filled with a living bone segment. Additional spongioplastics was required in 15 cases (75%), with additional internal osteosynthesis in 9 cases (45%), i.e. in 85% of cases plaster cast immobilisation was applied after previous removal of the external fixator. In the majority of cases (85%) this method included transfer of a microvascularised musculocutaneous flap. This method yielded good functional results, while in certain cases the aesthetic results were less satisfactory. Treatment according to the Ljubljana method is slightly more demanding due to a more difficult positioning of the traction system, but damage to soft tissues during the traction of the bone segment is lesser and the time required for traction is significantly shorter.
