Abstract     Bacground. Our assessment of prognosis in operable breast cancer patients and the choice of an appropriate therapy is based on prognostic factors. Promising prognostic factors include cathepsin D, a lysosomal protease and PS2, a protein of yet unknown function. The aim of this study was to determine the prognostic significance of Cathepsin D and PS2. Methods. 153 operable breast cancer patients treated at the Institute of Oncology in Ljubljana were included in our study. Levels of cathepsin D and PS2 were immunoradiometrically determined in frozen tumour cytosols. The median follow-up period was 33 months. Univariate statistical analysis (Log Rank test) was performed to evaluate the survival. Results. out of 153 patients included in the study 43 patients (28%) relapsed and 24 patients (16%) died. The overall survival of patients with cathepsin D levels<-50 pmol/mg prot. was significantly better (p = 0,03) than that of the patients with higher values. The difference in disease-free survival was not statistically significant (p = 0,09). The overall survival of patients with PS2 levels<-2,2 ng/mg prot was significantly better than that of the patients with PS2 levels<-2,2 ng/mg prot. (p = 0,02). The difference in disease-free survival was not statistically significant (p = 0,48). The overall survival of patients with PS2 levels<-2,2 (p = 0,02). Conclusions. Our univariate analysis has confirmed the prognostic value of cathepsin D and PS2 for overal survival but not for disease-free survival. Furthermore we have found that PS2 is particularly useful in the axillary lymphnode positive subgroup for the evaluation of overall survival.
Summary     Izhodišča. Za ločevanje različno potekajočih bolezni in izbiro zdravljenja se pri operabilnem raku dojk opiramo na prognostične dejavnike. Med obetavnimi sta katepsin D, ki je lizosomska proteaza, in PS2, protein neznane funkcije. Namen raziskave je bil ugotoviti prognostični pomen katepsina D in PS2 pri operabilnem raku dojk. Metode. V raziskavo je bilo zajetih 153 bolnic z operabilnim rakom dojk, operiranih na Onkološkem inštitutu v Ljubljani. Mediani čas nadzora ("follow-up") je bil 33 mesecev. na zamrznjenih citosolih tumorjev smo imunoradiometrično določili vrednosti katepsina D in PS2. Z univariatno statistično analizo (Log rank test) smo ocenjevali prognostični pomen katepsina D in PS2. Rezultati. pri 43 (28%), od 153 bolnic se je bolezen obnovila (relaps). Umrlo je 24 (16%) bolnic. preživetje bolnic z vrednostmi katepsina D 50 pmol/mg beljakovin ali manj je bilo statistično pomembno boljše od bolnic z vrednostmi, višjimi od 50 mol/mg beljakovin (p = 0,034). Za ozdravitev ta razlika ni statistično pomembna (p = 0,09). preživetje bolnic z vrednostmi PS2 več kot 2,2 ng/mg beljakovin je bilo statistično pomembno boljše od bolnic z vrednostmi PS2 2,2 ng/mg beljakovin ali nižjimi (p = 0,02). Za ozdravitev statistično pomembne razlike nismo ugotovili (p = 0,48). Preživetje bolnic s prizadetimi pazdušnimi bezgavkami je bilo statistično pomembno boljše pri vrednostih PS2 več kot 2,2 ng/mg prot (p=0,02). Zaključki. Katepsin D in PS2 v univariatni analizi statistično pomembno ločujeta bolnice glede na preživetje, ne pa glede na ozdravitev.
