Abstract     The method of motor evoked potentials (MEP) and its application in patients suffering from unilateral ischaemic stroke localised in the region of the middle cerebral artery. Forty patients were tested in the second week and 6 months after stroke. MEP of the biceps brachii, abductor digiti minimi, and tibialis anterior muscles were evoked by magnetic excitation of the motor cortex. The analysis of MEP showed changes suggesting axonal and/or cellular type of the corticospinal tract lesion and a reduction of corticospinal tract excitability. A follow-up study 6 months after stroke showed that evaluation of MEP in the early stage after stroke has prognostic value: the absence of MEP in the second week after stroke namely predicts unfavourable prognosis, while attenuated or norma MEP predicts good motor recovery.
