Title     Results of gastrectomies for gastric cancer: preliminary results

Abstract     In the period from January 1992 to July 1997 200 patients with carcinoma of the stomach underwent gastrectomy; there were 117 males and 83 females, their average age being 64.6 years. There were 113 distal resections (53%), 75 total gastrectomies (37.5%), 5 proximal resections (2.5%) and 7 stomach stump resections (3.5%) performed altogether. A curative RO resection was performed in 143 pts (71%), but in the remaining 57 pts (29%) only Rl or R2 palliative resection was possible. Out of 143 pts with RO resection 80 were distal resections (55.9%a), 55 total resections (38.5%), 2 proximal resections (1.4%) and 6 gastric stump resections (4.2%). Considering pts with RO resection, 85 had D2 lymphadenectomy and 58 D 1 lymphadenectomy. Of all 200 pts, 38 ( 19%) had complications ( 15 minor, 23 major). Average hospital stay was 14.6 days. The mortality rate was 7% for distal resections and 6.6% for total resections. The total mortality rate which included stump resections (3 deaths of 7 pts) was 8%. D2 compares favorably with D 1 lymhadenectomy in terms of 5-year survival for UICC stage (63% vs 35%a, p<0,05) and postoperative mortality (6.4% vs 6.8%).
