Abstract     Authors report a case of a 42 year old female patient with increasing jaundice and discomfort in the upper abdominal part. US and CT procedures showed a cystical change in the left liver lobe measuring 15 x 20 cm in diameter. ERCP showed extended and displaced biliary ducts below hepatic bifurcation and dilatation of biliary ducts proximal of the compression. US guided fine needle biopsy and cytologic examination of the sediment of aspirated fluid failed to confirm parasitic or malignant nature of the cystic liver change. During operation in the left liver lobe, multilocular cystic tumor was recognized and removed together with the gallblader. Histologic examination showed that the cystic tumor was hepatobiliary cystadenoma. Postoperative course was uneventful. Patient was discharged from the hospital on the 19th postoperative day. 12 weeks later, on follow-up examination, the pacient felt and looked well, with normal values of liver function tests and normal US examination.
