Abstract | Diarrhea represents a major health problem for travelers to tropical and subtropical countries. Since the syndrome is most often caused by an infection (>80%) acquired by ingestion fecally contaminated food or beverages, precautions regarding dietary habits remain the cornerstone of prophylaxis. Antimicrobials are a successful form of prophylaxis, recommended in specific cases. However, potential side effects of chemoprophylaxis can appear. The key factor in the management of acute watery travers' diarrhea is the restoration of water and electrolyte balance. Many patients will require no additional therapy, whereas some will need pharmacologic treatment to shorten the duration of diarrhea or to relieve the accompanying symptoms. Fluoroquinolone drugs have been shown to be the most effective in managing the travelers' diarrhea. Persons visiting tropical countries should be recommended to bring with them an antibacterial agent and an antidiarrheal drug (e.g. loperamid) as stand-by treatment. For children and the elderly oral rehydration solutions are indicated. With these drugs at hand, the traveler can face a trip with more confidence and fully enjoy stay in the tropics. | |
