Abstract     Interest in imported tropical diseases has increased with the rising number of travelers to the tropics. A persisting diarrhea (lasting 14 days) occurs in ca. 3% of travelers who have acute diarrhea on the trip. The causative organisms of chronic diarrhea are different from those causing acute diarrhea. Bacteria are relatively unusualy. Protozoa (Entamoeba histolytica, Giardia lamblia), helmints (Schistosoma mansoni/japonicum, Strongyloides stercoralis) or an opportunistic parasitic infestation associated with the HIV infection (Cryptosporidum parvum, Isospora belli, Cyclospora cayetanensis) are more likely. Furthermore, non-infestious causes, such as tropical sprue, secondary lactase deficiency or chronic inflammatory bowel disease have to be considered. At least three stool samples for examination for parasites, serological tests for parasites and a diet without lactose should be employed. Some patient need an uppeer and lower gastroenterologic workup. If no pathogen is defined an empirically antiparasitic and/or antimicrobial tratment can be given.
