Abstract     A short review of primary and secondary vasculitides is presented, together with the role and value of antineutrophil cytoplasmic antibodies (ANCA) in the diagnosis of ANCA-associated necrotizing crescentic glomerulonephritis (GN), and thedifferentialdiagnosis of the later. The idiopathic necrotizing crescentic GN probably should be reserved only for such rare cases of GN, with no evidence of ANCA, anti-GBM antibodies or immune complexes involvement. Moreover, the differences ofrenal morphology between 26 patients with ANCA against myeloperoxidase, and 13 patients with ANCA against proteinase 3 are disclosed. In the first group, segmental glomerular sclerosis and interstitial fibrosis are more frequent, while in the group, active lesions, such as are glomerular crescents and vasculitis, are presented.
