Abstract     The global and segmental left ventricular (l.v.) function was assessed before and 30 minutes after oral application of nicardipine using contrast quantitative cineventriculography. The study was performed on 10 patients with significant coronary artery disease (at least one more than 70 % stenosis proved by selective coronary angiography). The improvement of the global function was found in all of them: both ejection fraction (EF) and fiber shortening valocity (Vcf) increased substantially and statistical highly significant after nicardipine. The improvement of l.v. segmental function was statistically highly signifficauit (p=0001) in post-stenotic noninfarcted segments, moderately significant (p=0,03) in normal segments and nonsignificant (p=0,14) in infarcted myocardial segments. Accordingly, orally applicated nicardipine is a suitable drug for treataent of ischemia provoked contraction abnormalities in the coronary artery disease; highly significant improvement of contraction was namely found only in post-stenotic noninfarcted myocardial segments. The improvement of contraction was postulated to be due to vasodilatatory effects of this drug on arterial tree as well in systemic as in coronary circulation in ischemic segments. As described in literature, nicardipine has the lowest negative inotropic effect among all the calcium channel blockers. It is the only drug in this group which significantly improves the contraction of ischemic segments as early as 30 minutes after oral application.
