Abstract     Prognosis of patients with gastric cancer depends mainly on the spread of disease in the time of operation, which is best evaluated with pTNM stages. Depth of tumor invasion (pT) and involvement of regional lymph nodes (pN) of TNM classification are the most important independent prognostic factors for the outcome of patients who underwent RO resection with D2 lymphadenectomy. In cases when uncomplete D2 lymphadenectomy is performed precise pathohistological TNM staging is diffcult and the prognostic value of pN classification is not reliable because of the 'stage migration' phenomenon. In our study we have assesed the prognostic significance of the number of infiltrated lymph nodes as well as the ratio between the number of infiltrated lymph nodes and the number of resected lymph nodes for the outcome of patients who underwent RO resection with uncomplete D2 lymphadenectomy. Uni- and multivariate analysis of our 5-year series (290 patients) indicate that the ratio between the number of infiltrated lymph nodes and the number of resected lymph nodes is like the depth of tumor invasion (pT) independent prognostic factor also for the outcome of patients who underwent RO resection with uncomplete D2 lymphadenectomy. For these patients the number of infiltrated lymph nodes depends on the number of resected lymph nodes and because of the 'stage migration' phenomenon it is not reliable as the prognostic factor.
