Abstract     Background. Clonality of lymphoid proliferations is of important diagnostic value for distinguishing between neoplastic and reactive lesions. We wanted to introduce molecular genetic techniques in the diagnosis of lymphoproliferative disorders, since classic histopathologic techniques are not sufficient for clonality analysis. Determination of clonality can be achieved using methods based on the analysis of antigen receptor gene rearrangements. Methods. With the use of Polymearse Chain Reaction we amplified DNA sequences in rearranged genes coding for B cell and T cell antigen receptors. We distinguished between monoclonal and polyclonal lymphoid proliferations by electrophoretic separation of amplified fragments in polyacrylamide gels. Results. We have determined the clonality of 21 analysed B cell neoplastic lesions (100%) and 22 out of 30 analysed T cell neoplastic lesions (73.33%). Conclusions. We have established the method for determination of B cell and T cell clonality. Specificity and sensitivity of the method are in agreement with the published results.
Summary     Izhodišča. Klonskost limfoidnih proliferacij je pomemben diagnostični kriterij za ločevanje med neoplastičnim in reaktivnim procesom. Klasične histopatološke metode ne zadostujejo za analizo klonskosti, zato smo želeli uvesti molekularno genetske metode kot pomoč pri diagnostiki limfoproliferativnih procesov. Molekularno genetsko določanje klonskosti temelji na metodah, ki dokazujejo preureditve v genih za antigenske receptorje limfocitov B in T. Metode. preurejene genske sekvence, ki kodirajo posamezne verige receptorjev na površini limfocitov B in T, smo pomnožili z metodo reakcije verižne polimerizacije. nastale fragmente smo analizirali z elektroforezo v poliakrilamidnem gelu in tako ločili med monoklonsko in poliklonsko limfoidno proliferacijo. Rezultati. Določili smo klonskost 21 preiskanih limfoidnih neoplazem tipa B (100%) in 22 od 30 preiskanih limfoidnih neoplazem tipa T (73,33%). Zaključki. Uspešno smo vpeljali metode za določanje klonskosti limfoidnih proliferacij tipa B in T. Specifičnost in občutljivost metode so v soglasju s podatki, objavljenimi v literaturi.
