Abstract     Indirect calorimetry can be a useful tool to define nutritional status, determine nutritional requirements, and assess response to nutritional interventions. The metabolism of critically ill infants and children is significantly influenced by their underlying diseases, and standard predictive equations result in inappropriate nutritional support in most of these patients. Inadequate or excessive energy supply can adversely affect the clinical course of many critically ill patients. For these reasons, serial measurments of energy expenditure should be considered whenr accurate detemination of energy needs is deemed necessary. Measurements of oxygen consumtion and carbon dioxide production may be also used to determine cardiac output and work of breathing, and estimate the components of minute ventilation. An understanding of the potential technical and physiological pitfalls is necessary to obtain meaningful and useful ifnormation.
Summary     Indrektna kalorimetrija se uporablja za oceno prehranjenosti in določitev prehranskih potreb bolnika, z njo pa sledimo tudi učinkom naših ukrepov v prehrani. Bolezen kritično bolnih dojenčkov in otrok pomembno vpliva na njihovo presnovo, kar zadeva tudi ustrezno prilagajanje prehrane, tega pa ne moremo izvjati na podlagi ocene njihovih energetskih potreb, izračunanih s standardnimi enačbami. Nezadostni ali pretirani vnos hrane lahko škodljivo vpliva na potek zdravljenja kritično bolnih bolnikov. Večkratno merjenje energetske porabe je torej koristno, kadar potrebujemo natančno oceno energetskih potreb bolnika. Z merjenjem porabe kisika in tvorbe ogljikovega dioksida lahko posredno ocenimo tudi minutni volumen srca, dihalno delo in minutno ventilacijo. Za pridobitev natančnih in uporabnih rezultatov je nujno poznavanje možnih tehničnih in fizioloških zapletov med samim merjenjem.
