Abstract     We studied the effect of repetitive transcranial magnetic stimulation delivered to the motor cortex at 120% motor threshold intensity in 13 subjects at rest and in 8 subjects during a background contraction. A train of 20 stimuli delivered at a frequency of 5 Hz with th subjects at rest increased the size of muscle evoked potentials evoked in hand and forearm muscles; the facilitation was already evident over trains of 11 stimuli. The facilitation of the test response obtained after a conditioning train of stimli outlasted the trains by 600 ms. When the subjects voluntarly contracted the target muscle a train of 20 stimuli at 5 Hz left the size of the MEP unchanged. We conclude that repetitive transcranial magnetic stimulation changes motor cortex excitability by facilitating cortical excitatory interneurones.
