Abstract     We analysed motor cortex excitability during focal transcranial magnetic stimulation (TCS) in both affected (AH) and unaffected (UH) hemispheres of 20 monohemispheric stroke patientsevaluated at 2 (T1 session) and 4 (T2 session) months from stroke. AH was less excitablethan UH in T1 with an increase in T2. In AH, MEPs amplitude diminished in T1 improving in T2. In T2, when the excitability threshold (ETh) of AH improved, UH excitability decreased. It futher increased when the AH persisted to be unexcitable. This phenomenon was combined with clinical improvement. Recovery from AH hypoexcitability with progressive "balancing" of the UH excitability represents a possible prognostic parameter for clinical outcome.
