Abstract     Cold-pressor test (CPT) and head-up tilt (HUT) evoke an activation of the intracranial sympathetic nervous system (SNS). The response to CPT and HUT in patients with the dysfunction of autonomic nervous system (ANS) is stillnot clear. It is well known that patients with long duration of diabetes mellitus have high incidence of ANS impairment. To study the SNS response in diabetics with long history of diabetes mellitus (DM) and impaired ANS, we measured the mean arterial velocity in the middle cerebral arteries, using TCD, during CPT and HUT. In our study the values of MAP during CPT and HUT remained in autoregulatory range. We found significant lower values of vm changes indiabetic patients for CPT but not for HUT. Furthermore, multivariate analysis during CPT showed a CPT but not for HUT. Furthermore, multivariate analysis during CPT showed a significant correlation of vm changes as well as MAP changes with DM. SNS in diabetics with ANS dysfunction appears to be impaired. It seems that CPT is a more sensitive test for SNS in diabetic patients.
