Abstract     We performed preoperative and postoperative neuropsychological examinations in 24 patients who underwent carotid endarterectomies under SEP monitoring. The decisions to shunt or to restore pressure were taken on the basis of SEP alterations. Using this strategy no significant neuropsychological alterations were demonstrated postoperatively Although shunted patients had a significantly lower degree of ipsilateral stenosis, a trend toward a higher degree of contralateral stenosis, and a significantly lower stump pressure, none of these individual parameters were sufficient to predict the need for shunting.
