Abstract     Intraoperative monitoring of the corticospinal tract (CT) has a two decade long history. Due to the non specificity and non selectivity of the used previously methods for the intraoperative monitoring of the CT functional integrity, they should be abandoned. New methods of transcranial and direct motor cortex electrical stimulation with single and short train of stimuli, combined with recordings of Dewaves and motor evoked potentials from the muscle, should be used instead. The new methods have a solid theoretical background, and a high degree of correlation with motor outcome.
