Abstract     Background: It is not easy to decide on the mode of surgical treatment for female stress urinary incontinence (UI). The efficient outcome of surgical treatment depends not only on the surgeon's experience with a certain type surgery, but also on the type and degree of UI, changes of the genital statics, andaccompanying diseases. Nowadays, one of the most frequently applied surgeries in the treatment of stress UI is Burch colpourethrocystopexy. On the other hand, the suspension of the urinary bladder neck in stress UI can be performed endoscopically. Patients and methods: Since 1985 the Marshall-Marchetti-Krantz procedure has no longer been used at the Department of Obstetrics and Gynecology in Ljubljana, because of poorer postoperative results than achieved by Burch colpourethrocystopexy. We perform over 50 classical (open) Burch colposu-spensions yearly. Additionally, in 1995, we performed Burch extraperitoneal colposuspension using endoscopic approach and ballon distension of the space of Retzius in 5 patients suf-fering from stress UI. Results: Before surgery all patients had the diagnosis of stress UI confirmed by urodynamic testing. To evaluate the outcome of surgery, urodynamic testing were was repeated 6 and 12 months after surgery. One year after surgery the classical approach proved efficient in 99.13% (2 failed surgeries of the 230 performed), and endoscopic approach proved efficient in 80% (1 failed surgery of the 5 performed). Conclusion: Colpoure throcystopexy or Burch colposuspension is efficient surgical mode of treatment of stress UI. Short-term results are excellent.(Abstract truncated at 2000 characters.)
