Abstract     We studied how renal pathologists who were not among the authors of the Banff classification were reproducible in their utilization of the Banff criteria for histological grading of renal allograft rejection. Several sections from 15 renal allograft biopsies taken from 15 patients post-transplantation were stained with H&E, PAS and Msson's trichrome. They were examined by 4 independent renal pathologists without clinical data and according to the 1993 Banff scheme. Kappa coefficient, a measure of interosberver aggreement corrected for change agreement, was used for evaluating reproducibility. The agreement for scoring codes was fair for g and t but slight for i and v. The agreement was slight for g1, i1, i2, i3, t1, t2, t3, v0, v1, v2, v3, fair for g0, g2, moderate for i0, t0 and almost perfect for g3. The overall agreement for rejection was moderate. The agreement was substantial for no or borderline rejection, slight for grade I, and fair for grade II and III rejection. Discordance between codes and rejection grades reached up to 50%. In conclusion, although the overall agreement on the diagnosis of acute rejection was moderate, it seems that an effort should be undertaken to enhance reproducibility of i and v codes as well as of grade I and II rejection grade. This may be achieved by practise, tuition and a more accurate definition of grade I and II rejection, as this may be clinically important.
