Abstract     Aim. The aim of this study is through thesis of individual responds on foreign organ (transplanted kidney) and changes on blood vessels: a) prove a need of determination of Resistance Index (RI) value for each transplanted individual separately; b) determine a ragne of normal RI values and borderline to acute rejection (AR); c) determine a level of change of RI values in AR. Methods and Patients.From 1.1.1991. to 31.12.1997. in Ultrasound Unit and Dialysis and Transplantation Center-KBC Rijeka were followed 194 patients with transplanted kidney, in which RI values were measured with Duplex Color Doppler (DCD). Individuals are separated in two main groups: 1) examined from 1.1.1991. to 31.12.1991. and 2) examined from 1.1.1992. to universal values for normal and pathological stages were used and in second group our hypothesis of individual RI values for each patients separately was examined. Results. The difference between mentioned groups were showed through sensitivity, specificity, accuracy, positive and negative predictive values. Universal RI values range were 80-91% and with individual 88-96% in AR. Chi-square test with Yates correction showed statistically significant difference in AR (p=0,019). Conclusion. Our comparative analysis of universal RI values and new method of individual RI values usage present that individual RI values give accurate and earlier diagnosis in acute rejection.
