Abstract     Introduction of dialysis into the therapy of acute renal failure enables survival of patients without renal function who would otherwise die. Dialysis exerts a great support to kidney transplantation because it can keep patients alive also in the situation of failing or nonfunctioning graft. Acute graft failure, other than acute rejection, is due to many factors: acute tubular necrosis, vascular (thrombosis, stenosis of renal artery), hemodynamic (ACE inhibitors) and drug toxicity (cyclosporine) as the most important. Early diagnosis and appropriate therapy of acute graft failure can preserve renal function. The cornerstone of prevention and managementof acute tubular nercrosis in adequate hydration with saline solution. There are only few drugs which could be recommended in the prevention of acute tubular necrosis during transplantation. When dialysis in needed, procedures with low ultrafiltration rate and using synthetic membranes of the dialyser should be recommended.
